We recommend that you:
Bring your insurance card to every appointment.
Bring a copy of any important imaging including X-ray, MRI or CT scan.
Please note that co-pays, co-insurance, deductible amounts required by your
insurance company, are due at the time of your visit. We accept cash, check,
credit or debit card or FSA/HSA card.
Insurance Accepted and Payments:
We are in-network providers for most health insurance plans, including Dean Health Plan, SSM Health, Well-First, Quartz, Blue Cross/Blue Shield, Medicare, Medicaid, the Alliance, Care Wisconsin. We also accept Motor Vehicle Accidents and Worker’s Compensation. Please be sure to bring your insurance card with you on your first visit. We will collect all patient portion amounts at the time of service. We accept the following as forms of payment: Visa, MasterCard, American Express and Discover Card, checks and cash. We also accept online payments, through our portal online at www.zelmchiropractic.com.
